Official Entry Form

In signing this entry form each of the undersigned agrees to the following: (i) To release all other contestants, tournament officials, hosts, and sponsors for any and all damages, claims, demands or expenses relating to or resulting from this tournament, including personal injury and/or property damage. (ii) Not to sue or make any claim against any individual or group involved with this tournament now in the future concerning any injury or damage related to or resulting from this tournament. (iii) To indemnify and hold The 2 of Us and its officials or employees (hereinafter referred to as The 2 of Us) harmless from any liability whatsoever if The 2 of Us or its officials, or employees are involved in a lawsuit resulting from the undersigned's actions, regardless of whether intentional or accidental, and (iv) To accept the tournament director's decision as final in all matters pertaining to the tournament.

This signed release will also serve as notice that my insurance carrier will not sue or make claim against The 2 of Us and it's officials or employees for any claims they may pay on my behalf.

By signing this form I acknowledge that I understand that failure to take and/or clear one or more polygraph tests as requested by The 2 of Us will result in my disqualification with no refund or recourse against The 2 of Us. Further, I certify that I have read and understood all the rules and regulations concerning this tournament which are set forth in the "Official Rules". I also acknowledge that any misstatements or untruthful statements made by the undersigned, are my own personal responsibility.
